Both ideas seem so blindingly obvious, so utterly axiomatic for the self-involved baby boom generation of which I am part, that I?m still not sure why anyone needs to be reminded. But the numbers tell a different story.

An Anatomical Study on the Position of the Summit of the Zygoma: Theoretical Bases for Reduction Malarplasty

Background: To achieve optimal outcomes in reduction malarplasty, the area of zygoma from which volume should be reduced must be accurately identified. This anatomical study aims to evaluate the location of the zygoma region that contributes most to the protrusion of the cheek.
Methods: The morphology of the zygoma was studied on 121 Japanese adults (73 men and 48 women). The midpoint of the inferior orbital rim, zygomaticomaxillary junction, the junction between the frontal process and the zygomatic arch, and the lateral orbital rim were marked to be used as anatomical reference points. Then, a vertical plane intersecting the anterior and posterior edges of the zygoma was marked. The point of the zygoma most distant from the plane was defined as the summit of the zygoma. Three-dimensional measurement using graphic software was performed, and the positional relationships between the summit and the four reference points were evaluated.
Results: In terms of horizontal position, the summit is located lateral to the lateral orbital rim and medial to the junction between the frontal process and the zygomatic arch. Regarding vertical position, the summit exists at higher positions in men than in women.
Conclusions: The summit of the zygoma is located medial to the junction of the frontal process and zygomatic arch. Therefore, bone incision lines should be placed medial to the posterior edge of the frontal process in reduction malarplasty; effective correction of the protrusion cannot be achieved solely by detachment of the zygomatic arch. The summit is located higher for men than for women. Therefore, bone incision lines should be placed higher for men than for women.

Abstract
BACKGROUND: Excess body fat, localized adiposity, and cellulite represent important social problems. To date, techniques using radiofrequencies, cavitation and noncavitation ultrasound, and carbon dioxide have been studied as treatments for noninvasive body contouring. Ice-Shock Lipolysis is a new noninvasive procedure for reducing subcutaneous fat volume and fibrous cellulite in areas that normally would be treated by liposuction. It uses a combination of acoustic waves and cryolipolysis. Shock waves, used normally in the treatment of renal calculi and musculoskeletal disorders, are focused on the collagen structure of cellulite-afflicted skin. When used on the skin and underlying fat, they cause a remodeling of the collagen fibers, improving the orange-peel appearance typical of the condition. Cryolipolysis, on the other hand, is a noninvasive method used for the localized destruction of subcutaneous adipocytes, with no effects on lipid or liver marker levels in the bloodstream. The combination of the two procedures causes the programmed death and slow resorption of destroyed adipocytes. METHODS: In this study, 50 patients with localized fat and cellulite were treated with a selective protocol for the simultaneous use of two transducers: a Freezing Probe for localized fatty tissue and a Shock Probe for fibrous cellulite. RESULTS: The procedure significantly reduced the circumference in the treated areas, significantly diminishing fat thickness. The mean reduction in fat thickness after treatments was 3.02 cm. Circumference was reduced by a mean of 4.45 cm. Weight was unchanged during the treatment, and no adverse effects were observed. Histologic and immunohistochemical analysis confirmed a gradual reduction of fat tissue by programmed cell death. Moreover, the reduction in fat thickness was accompanied by a significant improvement in microcirculation, and thus, the cellulite. The safety of the method also has been highlighted because it is accompanied by no significant increase in serum liver enzymes or serum lipids. CONCLUSION: The study aimed to observe the effects of the new technique in the treatment of localized fat associated with cellulite in order to assess adipose tissue alterations, cellular apoptosis, and levels of serum lipid or liver markers. The findings show that the action of Ice-Shock Lipolysis is a safe, effective, and well-tolerated noninvasive procedure for body contouring. In particular, the authors believe that this could be an ideal alternative to liposuction for patients who require only small or moderate amounts of adipose tissue and cellulite removal or are not suitable candidates for surgical approaches to body contouring.

Currently, filling, smoothing, or recontouring the face through the use of injectable fillers is one of the most popular forms of cosmetic surgery. Because these materials promise a more youthful appearance without anesthesia in a noninvasive way, various fillers have been used widely in different parts of the world. However, most of these fillers have not been approved by the Food and Drug Administration, and their applications might cause unpleasant disfiguring complications. This report describes a case of foreign body granuloma in the cheeks secondary to polyethylene glycol injection and shows the possible complications associated with the use of facial fillers.

Guide to Early Surgical Management of Lip Hemangiomas Based on Our Experience of 214 Cases

Background: Hemangiomas are the most common vascular lesions in infancy. The majority of hemangiomas occur on the face and cause significant parental distress because of their high visibility. Traditionally, they have been managed by a ?watch and wait? approach due to their known propensity to improve significantly over time. This is a particularly difficult approach for hemangiomas of the lips due to their high visibility and their tendency to leave residual deformity even after resolution.
Methods: The authors performed a retrospective chart review of pediatric patients who underwent surgical excision of upper and lower lip hemangiomas at Children’s Hospital Los Angeles between 1992 and 2006.
Results: The authors identified 214 patients with an average age of 23.6 months at the time of surgery. All patients had improvements in lip appearance, with minimal complications. No transfusions were required, and all but two patients (lip switch flaps for major tissue loss) were managed on an outpatient basis. Overall, complications were minimal, with a high degree of parent satisfaction.
Conclusion: The authors conclude that many children would benefit from early excision and present guidelines for the early surgical management of lip hemangiomas.

Improved ?Bell-Bottom? Flap Surgical Technique for Syndactyly without Skin Graft

Background: This study describes the use of a modified ?bell-bottom? flap for treatment of partial syndactyly release that may avoid the use of skin grafting.
Methods: A retrospective review of this procedure was performed for 12 interdigital space reconstructions. Patients were aged 1 to 5 years 6 months. The mean follow-up period was 24 months. The operations were performed to avoid skin grafting. The flap was designed in the narrow-spaced and fused interdigital region of syndactyly for preventing postoperative scar contracture and web formation.
Results: Surgery was completed without skin grafting in all cases. The use of a modified flap allowed the construction of interdigital spaces with sufficient width, providing satisfactory cosmetic outcomes. No partial necrosis or complications were observed. No web formation, elevation of the interdigital space, or limited range of joint motion caused by contracture was observed during the 2-year follow-up period, and no secondary correction was needed.
Conclusion: The present surgical technique may be a new surgical option for web release in which fingers are fused at a level lower than the midpoint between the proximal and distal interphalangeal joints.

Every time Cultural Tourism DC presents its annual smorgasbord of free walking tours all over the city, I try to figure out how many I can manage to pack into just two days.

But this year I don?t have to do that ? because the WalkingTown DC/BikingTown DC program expanded from a single weekend to nine days. It started this past weekend and continues through Sunday, presenting nearly 200 chances to take a scholarly stroll (or roll, in the case of the dozen bike tour options) through all eight of Washington?s wards. To keep numbers manageable, some popular walks require advance reservations, but most of them are open to anyone who shows up. For every tour, the schedule lists fitness level, which is calculated based on the distance and terrain. Options range from a few blocks to several miles.

Abdominoplasty with Suction Undermining and Plication of the Superficial Fascia without Drains: A Report of 113 Consecutive Patients

Background: Postoperative abdominoplasty seromas are a problem. Although drains are still commonly used during the initial postoperative period, this article has demonstrated that the combination of an extended incision, suction undermining, and progressive tension sutures can produce superior results without the need for suction drains.
Methods: A retrospective review of 113 consecutive abdominoplasty patients operated on between April of 2004 and May of 2010 was carried out and complications were reviewed.
Results: There were 109 women and four men, with ages spanning 23 to 76 years (average, 50 years). Complications of the surgery included hematoma (2.7 percent), with one requiring drain placement (0.9 percent) and two treated with needle aspiration (1.8 percent); seroma (8.8 percent), with four requiring closed suction drainage (3.5 percent) and six minimally treated with needle aspiration (5.3 percent); infection (2.7 percent), with one requiring intravenous antibiotics (0.9 percent) and two with minimal local erythema (1.8 percent); and minimal marginal skin necrosis with spontaneous healing (3.5 percent).
Conclusions: The technique of abdominoplasty with the addition of an extended incision, liposuction undermining of the deep fatty tissue between the superficial and abdominal muscle fascia, and the use of progressive tension sutures results in a better abdominal wall and waist contour. This decreases the need for dissection of the abdominal panniculus above the umbilicus except for a small tunnel to allow for the suturing of the rectus abdominis muscles. This allows for preservation of the arterial and lymphatic vessels, improving blood flow to the superior flap and decreasing seroma formation to the point where operative drains are not required.

Indocyanine Green?Enhanced Lymphography for Upper Extremity Lymphedema: A Novel Severity Staging System Using Dermal Backflow Patterns

Background: Management of arm lymphedema following breast cancer treatment is challenging, and emphasis should be put on early diagnosis and prevention of secondary lymphedema. Indocyanine green lymphography is becoming a method of choice for evaluation of lymphedema.
Methods: Twenty patients with secondary arm lymphedema after breast cancer treatment underwent indocyanine green lymphography. Characteristic findings of indocyanine green lymphography were analyzed according to corresponding clinical stages and duration of edema. Based on changes in indocyanine green lymphography findings with progression of lymphedema, a new severity stage, arm dermal backflow stage, was developed and compared with clinical stages.
Results: The indocyanine green lymphographic findings were classified into two large groups: linear pattern and dermal backflow patterns. The dermal backflow pattern could be subdivided into splash, stardust, and diffuse patterns. The dermal backflow patterns were found more frequently than the linear pattern in the proximal upper extremity (p = 0.001). The dermal backflow patterns also increased significantly in prevalence overall as the duration of lymphedema increased (p = 0.032). The arm dermal backflow stage was linearly correlated with clinical stage as described by the line y = 1.092x + 0.083 (R2 = 0.997; analysis of variance, p < 0.001).
Conclusions: Indocyanine green lymphography is a safe and convenient evaluation method for lymphedema that allows qualitative pathophysiologic assessment of lymphedema. The arm dermal backflow stage, based on indocyanine green lymphographic findings, is a simple severity staging system that demonstrates a significant correlation with clinical stage. Indocyanine green lymphography may come to play an important role in early diagnosis of secondary arm lymphedema.

The Possibility and Potential Feasibility of Putting an Extra Functioning Free Muscle Transplant onto a Normal Limb: Experimental Rat Study

Background: Functioning free muscle transplantation is currently and popularly used clinically to restore motor deficit caused by traumatic muscle loss or chronic muscle denervation. However, no one uses functioning free muscle transplantation in a normal subject who has no motor deficit. This study was designed to investigate the possible and potential feasibility of transferring an extra free muscle transplant onto a normal limb.
Methods: A chimeric flap including biceps muscle with its neurovascular pedicles and a skin perforator flap was designed, harvested, and transferred from a Lewis inbred rat to the other rat overlying the original biceps following neurovascular repair where the ulnar nerve was chosen as the neurotizer. Sixteen rats were operated on and evaluated 6 months after surgery. Outcome measurements included arm circumferences, electrophysiologic studies, elbow flexion force, and muscle mass. The contralateral normal biceps was used as the control.
Results: All outcome measurements revealed that the extra muscle transfer resulted in significant increases in size and function of the operative limb without interfering with the original biceps function.
Conclusion: The authors’ study demonstrates the possible and potential application of using extra free muscle transplantation for functional and aesthetic augmentation purposes in a normal subject.

Abstract
PURPOSE: Brain abnormalities in patients with syndromic craniosynostosis can either be a direct result of the genetic defect or develop secondary to compression due to craniosynostosis, raised ICP or hydrocephalus. Today it is unknown whether children with syndromic craniosynostosis have normal brain volumes. The purpose of this study was to evaluate brain and ventricular volume measurements in patients with syndromic and complex craniosynostosis. This knowledge will improve our understanding of brain development and the origin of raised intracranial pressure in syndromic craniosynostosis. METHODS: Brain and ventricular volumes were calculated from MRI scans of patients with craniosynostosis, 0.3 to 18.3 years of age. Brain volume was compared to age matched controls from the literature. All patient charts were reviewed to look for possible predictors of brain and ventricular volume. RESULTS: Total brain volume in syndromic craniosynostosis equals that of normal controls, in the age range of 1 to 12 years. Brain growth occurred particularly in the first 5 years of age, after which it stabilized. Within the studied population, ventricular volume was significantly larger in Apert syndrome compared to all other syndromes and in patients with a Chiari I malformation. CONCLUSIONS: Patients with syndromic craniosynostosis have a normal total brain volume compared to normal controls. Increased ventricular volume is associated with Apert syndrome and Chiari I malformations, which is most commonly found in Crouzon syndrome. We advice screening of all patients with Apert and Crouzon syndrome for the development of enlarged ventricle volume and the presence of a Chiari I malformation.
